Who Is Eligible for ECM Services? -

ECM is for eligible Medi-Cal members who need extra support with their health and daily needs.

• Experiencing homelessness or housing instability

• Seniors with complex health or social needs

• Involved with the justice system or returning home after incarceration

• Living with mental health or substance use challenges

• Managing complex health conditions

• Having difficulty getting the care or services they need

• Facing other challenges that affect their health and stability
